Care in your Care Home
At ellenor, our dedicated team provides specialist care and support in care homes for carers. Our aim is to provide carers with the necessary physical and practical support to ensure that patients in care homes are as comfortable as possible when nearing the end of life.
Who is the Care Home team and what do they do?
Our dedicated Care Home Team are experts in what they do. All of our staff have the necessary training and expertise in the provision of practical, appropriate and sensitive end of life care for both residents and carers that require support. Working closely in Partnership, the team at ellenor offer expert support to carers, enabling them to care for the residents in their care home at the end of their life.
Access our care and services
To receive care at your care home, you'll need to be referred to ellenor by a General Practitioner or Healthcare Professional.
To be referred you must:
- Have, be caring for, or be part of the family of someone with a life limiting condition.
- Agree to the referral.
There are a number of different reasons for referral, including pain control, psycho-social support, spiritual support and end of life care. We will request details of medication taken from the appropriate care professional.
